Custom Contact Form Included in Theme

Halo,
saat ini saya sedang membuat custom contact form tanpa plugin alias langsung terintegrasi di dalam file tema. namun ketika mengkik tombol kirim, pesan dari contact form itu tdk terkirim. ada yang bisa bantu mencari tahu apa kesalahan dari kode saya dibawah ini? terima kasih sebelumnya
